REFUND POLICY
Clients who have purchased weight loss packages including, but not limited to vitamins/supplements Smart Scale Body Scanner, and diet plans, will not be refunded for such packages if they decide not to continue with the program. Refunds for weight loss programs/packages will not be given under any circumstances. All Sales on weight loss programs/packages are final.
RESULTS ARE NOT GUARANTEED. RESULTS WILL VARY BASED ON EACH PATIENT’S PHYSICAL HEALTH, FAMILY HISTORY, DIET AND EXERCISE, PHYSICAL CONDITION AND ADHERENCE TO THE WEIGHT LOSS PROGRAM.
Please note that BEST WEIGHT WILL NOT SATISFY ANY REFUND/RETURN/EXCHANGE REQUESTS AND no refund WILL BE HONORED on purchase of any weight loss products/packages.
Under no circumstances can Best Weight refund patients for any services already received, including weight loss packages, vitamins/supplements, and Smart Scale Body Scanners.
By signing below, you confirm that as a customer you fully understand the above no refund policy, and are fully informed of Best Weight’s company regulation policies upon purchasing.

WAIVER OF CHARGE BACK / DISPUTE AGREEMENT
I. Purchase of W8 BAR and/or TRI HEALTH MEDICAL INSTITUTE product(s) service(s), (hereinafter PURCHASER), hereby W8 BAR and TRI HEALTH MEDICAL INSTITUTE to immediately bill my credit card for product(s) and/or service(s) that was ordered upon submitting my credit card information. PURCHASER stipulates to the following:

1. That, by submitting and order, agrees to this CREDIT CARD AUTHORIZATION AND WAIVER OF CHARGE-BACK/DISPUTE AGREEMENT whether purchase was made via credit card.
2. This authorization shall remain valid until written notice of cancellation is issued by PURCHASER and served upon and W8 Bar and Tri Health Medical Institute Notice of Cancellation does not supersede previous purchases made prior to cancellation whether purchase was made via credit card.
3. PURCHASER understands and agrees that this charge is final and strictly a NON REFUNDABLE purchase from W8 Bar and Tri Health Medical Institute and ALL item(s) service(s) are sold as is whether purchase was made by credit card and PURCHASER waives all rights and remedies afforded to him what-so-ever whether purchase was made via credit card.
4. PURCHASER waives any and all credit card charge-back rights and any and all dispute options for this purchase. By submitting an order pursuant to stipulation I constitutes authorization and agreement with this CREDIT CARD CHARGE AUTHORIZATION AND WAIVER OF CHARGEBACK/DISPUTE AGREEMNT which is given without reservation of any and all rights claimed, PURCHASER agrees that full disclosure was made as it relates to the product(s) service(s) purchased and agrees not to charge-back or dispute the charge whether purchase was made via credit card.

Best Weight Nutritional Informed Consent
According to the Federal Food, Drug, and Cosmetic Act, as amended, Section 201 (g) (1), the term “DRUG” is defined to mean: “Articles intended for use in the Diagnosis, Cure, Mitigation, Treatment or Prevention of disease.” A vitamin is not a drug, NEITHER is a Mineral, Trace Element, Amino Acid, Herb, or Homeopathic Remedy. Although a Vitamin, a Mineral, Trace Element, Amino Acid, Herb or Homeopathic Remedy may have an effect on any disease process or symptoms, this does not mean that it can be misrepresented, or be classified as a drug by anyone. Therefore, please be advised that any suggested nutritional advice or dietary advice is not intended as a primary treatment and/or therapy for any disease or particular bodily symptom.
Nutritional counseling, vitamin recommendations, nutritional advice, and the adjunctive schedule of nutrition is provided solely to upgrade the quality of foods in the patient’s diet in order to supply good nutrition supporting the physiological and biomechanical processes of the human body.
